What We Do
We have a range of different services and are able to offer intensive, coordinated home support for patients with complex needs who wish to stay in their own home. Patients tell us that the Hospice of the Valleys is an invaluable source of compassion, support and advice at a difficult time in their life and through the ‘I Want Great Care’ surveys (the rate and review website for the UK medical profession) patients tell us time and again that they value this service highly and wouldn’t change a thing. One of many similar comments received reads: “The care they gave my husband and support to me and my young children was amazing. And it’s nice to know that even though he passed away that there is still support there for me and my girls.”
Caring for over 500 existing patients and new referrals every year, with more than 12,000 patient and family “contacts” made by the multi-disciplinary team, bereavement support and specialist “out of hours” services, the Hospice is able to deliver a level of community support that consistently enables more than 70% of patients known to Hospice of the Valleys to achieve death in their preferred place of care, compared to a national average of under 25%.
Hospice of the Valleys provides care for adults aged 18years and above, however, in exceptional circumstances, where there is a proven case for adult specialist palliative care input, provision of care can be extended to the care of young adults, aged 16 – 18years, with the agreement of the Clinical Services Director and Palliative Care Consultant.
