Some people associate the term “hospice” with a building, but “hospice care” is not just a building, it is a way of caring for people who have a life-limiting diagnosis; it is an approach that improves the quality of life for patients and their families facing the problems associated with life-limiting illness, through the prevention and relief of suffering.


Our team of Clinical Nurse Specialists (CNS) provide direct support and advice to patients and their families and acts as an expert resource to other professionals in the community.

Any life limiting condition can be frightening for patients and carers and it is important to know that the nurse is there for support at each stage of the illness and where necessary, through to end of life care.

This is a specialist nursing service and you can feel confident discussing with them any issues or difficulties you may be having, including:

• Symptom management
• Psychological and emotional support for you and your carers
• Support through treatment
• Spiritual support
• Financial advice

We have a range of different services and are able to coordinate home support for patients with complex needs who wish to stay in their own home, including specialist nurse advice which is available 24 hours a day, 7 days a week, 365 days a year.

The nurse will discuss details of your care with other healthcare professionals only with your consent, but will work closely with colleagues in the NHS, e.g. your GP and District Nurse, or refer you to other members of the Hospice clinical team, to ensure that you receive the best possible care at all times.

Patients are provided with the contact details for their CNS team and as this service is provided 24hours a day 7days a week – an “out of hours” contact number.

The primary concern of the Clinical Team at Hospice of the Valleys is to bring the highest standards of care to patients and their families. In palliative care there is never “nothing more that can be done”. Our whole team work hard to make sure that patients and families get the care and support they need at the most difficult and stressful times of the illness journey.
